Pledge Form for the First Swim For Life 1988

This is a pledge form for the 1988 "Swim FOR Life" which Artist Jay Critchley created to Benefit the PWA Coalition, and later also Provincetown AIDs Support Group, Helping Our Women, and others. The Swim for Life, whereby people swim from Long Point across Provincetown Harbor to the bay beach, has become a yearly fundraiser donating much needed funds to local non-profit organizations.
